Question 1

How do I appeal an insurance claim denial?

Accepted Answer

To appeal an insurance claim denial, review your denial letter for the specific reason, gather supporting documentation (medical records, physician letters, policy terms), write a formal appeal letter addressing each denial reason, and submit it within your insurer's appeal window — typically 30–180 days. AppealIQ AI automates this entire process.

Question 2

What is the success rate for insurance appeals?

Accepted Answer

Internal insurance appeals succeed roughly 40–60% of the time when properly documented. External independent reviews overturn insurer decisions about 40% of the time. AI-assisted appeals with complete documentation significantly improve these odds.

Question 3

How long does an insurance appeal take?

Accepted Answer

Urgent/expedited appeals must be decided within 72 hours. Standard internal appeals take 30–60 days. External independent reviews take up to 45 days. Total resolution from denial to final decision typically ranges from 2 weeks to 6 months.

Question 4

Can I appeal a prior authorization denial?

Accepted Answer

Yes. Prior authorization denials are among the most commonly overturned. You can request an internal appeal, an expedited review if urgent, or an external independent review. A physician support letter citing medical necessity dramatically improves success rates.

Question 5

Is AppealIQ AI free to use?

Accepted Answer

Yes. AppealIQ AI is free to start. You can generate your appeal letter and review your denial analysis at no upfront cost.
